Rehabilitation Worker Ii (day and Evening Treatment Program)
About This Role
Shifts: 7-day rotating schedule (10- and 11-hour shifts including weekdays and alternating weekends)
The Rehabilitation Worker II will work with the client on an individual Recovery Plan of Care to address client goals. This position connects with clients living with substance use and receiving services through the Day and Evening Treatment program.
Responsibilities and Duties:
Work as a member of the multidisciplinary team to provide psychosocial rehabilitation services to clients with addiction and mental health disorders.
Supervise clients living in the community with substance use concerns by promoting a healthy living environment.
Develop and implement individual routines and programs to support the skill development of clients in accordance with their individual recovery plan.
Oversee the upkeep of the supportive housing unit to address health and safety of clients.
Assist clients with developing a monthly budget.
Encourage client participation in social recreation, vocation, education and wellness programs offered through LLGAMH and other community partners.
Monitor client compliance with medication regimes.
Participate in team meetings.
Provide education to clients regarding home safety practices.
In consultation members of the team, assist the client in attending scheduled appointments.
Assist clients who share a supportive housing unit to resolve conflicts as they arise. Internal applicants will be considered before external applicants. We thank all applicants for their expressed interest; however, only applicants selected for an interview will be contacted.
Respond to crisis situations as they arise.
Complete client urine screens.
Facilitate psycho-educational groups.
Qualifications & Requirements
Qualifications:
Education, Training and Experience:
A minimum of a college diploma in a health-related discipline having worked 2 years in mental health and addictions.
Course/workshop in concurrent disorders or equivalent (SMART Recovery Facilitator Training) preferred
Workshop in motivational interviewing preferred,.
Knowledge of, and experience with, crisis intervention and de-escalation (CPI Certificate)
Current First Aid and CPR Certification.
Satisfactory Vulnerable Sector Check is required.
A valid Ontario Driver’s Licence, access to a vehicle, and vehicle insurance with a minimum of $2,000,000 liability.
Skills and Abilities:
Understanding of Mental Health Act, Occupational Health & Safety Act, Ministry of Health and Long-Term Care guidelines, Psychosocial Rehabilitation Practitioner’s Code of Ethics, Best Practices, LLGAMH program standards.
Ability to work independently and interdependently.
Strong problem-solving abilities.
Excellent time management skills and ability to organize workload efficiently.
Requires analytical skills to gather and disseminate data from different sources.
Knowledge of community resources.
Ability to adapt to changing environments.
Computer competency (Le. MS Word, Excel, and various databases).
