A provincial initiative, the HART Hubs are healthcare facilities that provide an entire continuum of care under one roof, encompassing addiction treatment, mental health care, primary health care, supportive housing, vocational training, and social services to help people achieve long-term recovery and stability.
The HART Hubs DO NOT provide supervised consumption services, safer drug supplies, or needle exchange services.
HART Hubs require a referral and are for any individual who needs mental health and addiction support. Individuals may move through the full continuum of care or access only one or a few of the services (e.g. withdrawal management and addiction treatment only).
Some HART Hub clients are experiencing homelessness, and the treatment and wraparound services (e.g. transitional supportive housing, vocational training) help provide them with a pathway out of living on the streets.
Led by Lanark, Leeds and Grenville Addictions and Mental Health (LLGAMH) in partnership with more than 50 community organizations, the LLG HART Hub creates a coordinated pathway for individuals from withdrawal support to long-term recovery and stability. The HART Hub operates in two locations – one in the United Counties of Leeds and Grenville and one in Lanark County – ensuring that individuals move through different stages of treatment and housing without experiencing the instability that often leads to relapse.

The funding for the LLG HART Hub is currently pending approval by the Ministry of Health.
Referral information will be shared once the HART Hub has launched.
The HART Hub client services are publicly funded by the Ontario government. There is no additional cost to clients.
No, it will not be a gathering place in our community. Just like a hospital, the HART Hub will be a centre where clients are referred to receive care and progress along their healing journey. It will be staffed 24/7 by highly skilled addictions and mental health professionals who are trained in conflict de-escalation.
LLGAMH will continue to operate the successful Drop-In Centre at 25 Front Ave. that offers clients programming and support to address their basic needs, help with navigating resources, and a place to build community. The Drop-In will continue welcoming clients seven days a week (including holidays).
When the HART Hub locations are confirmed, Lanark Leeds and Grenville Addictions and Mental Health will host public information sessions in the United Counties of Leeds and Grenville, and Lanark County.
