What is a HART Hub?

A provincial initiative, the HART Hubs are healthcare facilities that provide an entire continuum of care under one roof, encompassing addiction treatment, mental health care, primary health care, supportive housing, vocational training, and social services to help people achieve long-term recovery and stability.

 

Do HART Hubs provide safe supply?

The HART Hubs DO NOT provide supervised consumption services, safer drug supplies, or needle exchange services.

 

Who is eligible to receive care and support from HART Hubs?

HART Hubs require a referral and are for any individual who needs mental health and addiction support. Individuals may move through the full continuum of care or access only one or a few of the services (e.g. withdrawal management and addiction treatment only).

Some HART Hub clients are experiencing homelessness, and the treatment and wraparound services (e.g. transitional supportive housing, vocational training) help provide them with a pathway out of living on the streets.

 

How will the Lanark, Leeds and Grenville (LLG) HART Hub serve the region?

Led by Lanark, Leeds and Grenville Addictions and Mental Health (LLGAMH) in partnership with more than 50 community organizations, the LLG HART Hub creates a coordinated pathway for individuals from withdrawal support to long-term recovery and stability. The HART Hub operates in two locations – one in the United Counties of Leeds and Grenville and one in Lanark County – ensuring that individuals move through different stages of treatment and housing without experiencing the instability that often leads to relapse.

 

What services does the LLG HART Hub offer?

The HART Hub offers a seamless, multi-phase approach to addiction recovery. Our program is designed to support individuals through every stage of the healing journey - from withdrawal management to long-term stability in the community.

1. Withdrawal Management (3 to 14 Days) 

Clients begin their recovery with medically supported withdrawal in a safe and caring environment.

  • Daily contact with a Nurse Practitioner and Registered Practical Nurses
  • Use of medications to ease withdrawal symptoms and ensure safety
  • As soon as physically able, clients begin:
    • One-on-one sessions with an Addictions Counsellor
    • Meetings with a Case Manager
    • Participation in group programs if health permits

2. Intensive Residential Treatment (3 months)

Once withdrawal is complete, clients transition into our Intensive Residential Treatment program. This phase includes:

  • 5 - 6 hours of daily programming, combining group and individual counselling and therapy
  • Topics covered in groups include:
    • Cognitive Behaviour Therapy (CBT) for Anxiety and Depression
    • Coping with Trauma
    • Anger and Addiction
    • Substance Use Recovery 101
    • SMART Recovery
    • Mindful Recovery
    • Relapse Prevention
    • Relationships in Recovery
  • Vocational supports to help prepare for work, school, or volunteering after treatment

3. Supportive Treatment (3 Months)

Clients who complete intensive treatment can move into a supportive treatment phase with a focus on reintegration and skill-building.

  • Less intensive daily structure
  • Continued substance use programming through daily groups and individual counselling
  • Emphasis on:
    • Employment readiness
    • Job skills training
    • School credit upgrading
    • Volunteer placements

4. Supportive Housing

The final stage of care provides clients with stable housing in a substance-free environment for up to 2 years while they fully reintegrate into the community.

  • Opportunities for:
    • Part-time or full-time work 
    • Volunteering
    • School credit upgrading or college-level study
  • Continued support to maintain recovery and build independence

 

Who are the staff supporting HART Hub clients?

The HART Hub team of mental health and addictions professionals includes:

  • Nurse Practitioners and Registered Practical Nurses providing medical supervision and withdrawal management support
  • Addiction and mental health specialists offering individual and group therapy
  • Case managers and social workers assisting with housing applications, employment readiness, and long-term recovery planning
  • Rehabilitation workers assisting clients with activities of daily living and providing support and guidance as they progress through the various levels of the HART Hub
  • Peer support workers and individuals with lived experience providing guidance and encouragement and help clients on their journey toward stability
  • Recreational counsellors leading wellness programs and community reintegration activities

 

What are the two locations of the Lanark Leeds Grenville HART Hub?

Lanark location: 9 Armstrong Drive, Smiths Falls 

Leeds Grenville location: 280 King St E, Brockville

 

 

What is the rationale for choosing these specific locations?

The HART Hub locations will meet the following criteria:

  1. Proper zoning
  2. Appropriate facility space and layout
  3. Close to the priority populations who will access services

 

Who funds the LLG HART Hub? Has a long-term funding commitment been made?

The Ministry of Health funds the LLG HART Hub. They have made a long-term funding commitment.

 

How can individuals access LLG HART Hub services?

The process for HART Hub referrals and self-referrals is being finalized and will be announced later in fall 2025.

 

Is there a cost to access the LLG HART Hub services?

The HART Hub client services are publicly funded by the Ontario government. There is no additional cost to clients.

 

Will the HART Hub be a gathering place in the community?

No, it will not be a gathering place in our community. Just like a hospital, the HART Hub will be a centre where clients are referred to receive care and progress along their healing journey. It will be staffed 24/7 by highly skilled addictions and mental health professionals who are trained in conflict de-escalation.

LLGAMH will continue to operate the successful Drop-In Centre at 25 Front Ave. that offers clients programming and support to address their basic needs, help with navigating resources, and a place to build community. The Drop-In will continue welcoming clients seven days a week (including holidays).
